SUMMARY Under general supervision, is responsible for providing medical treatment, first aid, emergency care and medications, under the direction of a Registered Nurse and in accordance with ADAPT Community Network (ADAPT) policies, the regulations and requirements of applicable regulatory agencies and the standards of the profession. This position is in one of our Brooklyn Residential ICF's Sunday, Monday, and Tuesday from 9AM-9PM. ESSENTIAL DUTIES AND RESPONSIBILITIES The duties and responsibilities of the Licensed Practical Nurse - Residential will include, but are not limited to the following: * Serves as a member of interdisciplinary team and is responsible for assisting Registered Nurse in coordinating following through on provisions of health care services for the assigned residence. * Keeps progress notes under supervision of registered nurse for each client and summarizes for quarterly, semi-annual and annual reviews. * Provides health care to individuals receiving services as instructed by registered nurse. Provides direct hygiene and health instruction to individuals receiving services and families as required. * Handles control of communicable diseases and infections through identification and assessment, reporting any finding to registered nurse and consulting physician, implementing appropriate protective and preventive measures and monitoring infection control practices. * Provides first aid to persons in need of immediate care, with referral to registered nurse or outside emergency services as necessary. * Assists physicians in medical examinations as needed. * Maintains a tickler for all appointments. Schedules appointments and follows up on physicians recommendations. * Keeps up to date and accurate records of examinations, immunizations, illnesses, accidents and other pertinent data via progress notes. * Administers medications when appropriate. Supervises storage of medications and assists in oversight and ordering of medication. * Assists registered nurse in maintaining AMAP re-certification files. * Obtains consent for surgery of residents in absence of registered nurse. * Refers all consults and diagnostic reports to registered nurse for review. * Attends and transports clients to appointments as necessary. * Attends team meetings as scheduled. * Attends to clients medical conditions. * Carries work phone and is on-call, responds to questions regarding physician and RN treatment plans. * Keeps abreast of changes in Nursing field. * Performs other related duties as requested. QUALIFICATIONS To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. EDUCATION and/or EXPERIENCE * Graduate of an accredited program of professional nursing * At least one year of experience as a Licensed Practical Nurse in the delivery of community-based health services to individuals with developmental disabilities. * Experience working with high-medical needs, e.g., g-tubes & tracheostomy tubes, is a plus! LICENSE/CERTIFICATION * License and current registration as a Licensed Practical Nurse in the State of New York * Valid New York State Drivers License and maintain it in good standing. * CPR certified SCHEDULE: Sunday, Monday, and Tuesday - 9AM to 9PM COMPENSATION: $38 - $43/hour + Industry Leading Benefits!
