*PSYCHIATRIST Nurse Practitioner /_PNP**Program: YOUNG ADULT ASSERTIVE COMMUNITY TREATMENT ACT *Young Adult Assertive Community Treatment (YAACT) is an evidence-based, mobile practice model designed to provide treatment, rehabilitation and support services to individuals living with a serious mental health condition and whose needs have not been effectively met by traditional mental health services.ACT teams are multi-disciplinary and include psychiatry, nursing, social work, peer specialists and other staff members offering rehabilitation, treatment and recovery support services. The team provides expertise in the areas of housing, substance use and harm reduction, family support, employment, and wellness self-management. Based on their area of expertise, team members collaborate to deliver comprehensive, integrated and flexible services that are responsive to a participant's choices, needs and goals. With a small staff to participant ratio, services are brought to the participant and offered in natural settings, 24 hours a day and seven days a week.The team NPP provides clinical leadership, working closely with the Team Leader, providing treatment to individuals within their community especially to those who choose not to present to the office.*Essential Job Functions** Provide psychiatric and medical assessment and treatment.* Conduct comprehensive psychiatric assessments and develop holistic service plans addressing the social determinants of health.* Monitor treatment and provide follow up visits, treatment for co-occurring substance use disorders and respond to crisis situations.* Serve as the medical leader for the team, overseeing team delivery of clinical services, participating in team meetings to evaluate risk and risk management needs.* Provide coordinated care, collaborating with family and other community treatment providers, nursing staff and Team Leader.* Deliver integrated care using best prescribing practices.* Provide clinical supervision, education, and training of the YAACT team members.* Develop, maintain, and supervise medication administration policies and procedures.* Develop, implement and revise psychiatric and medical treatment policies procedures with support from Team Leader.* Document services and changes in medication or treatment plans and consistent with agency and regulatory and licensing entities.* Work in collaboration with YAACT Team Leader, assuming leadership in his/her absence.* Provide 24/7 on-call consultation*Job Qualifications** Licenses: MHPMHNP/CRNP in psychiatric mental health for New York State* _*Required*_ Certification by The American Nurses Credentialing Center (ANCC/ANA)* _*Required *_Active and valid DEA* Commitment to person-centered treatment strategies, upholding participants' Eligible for full and unconditional participation in the Medicaid and Medicare*SALARY: $95,000-$105,000**BENEFITS WE OFFER: *• Medical including Dental and Vision• Cafeteria Plans: FSA, Dependent child care plan, qualified transportation plan• Up to 3% 401k Profit Sharing plan automatic contribution for eligible employees• 401k Additional 4% match of your contribution from your salary• Employee Assistant Program Services (EAP)• Community Mental Health Loan Repayment Program for qualifying candidate• Paid time off (PTO)• Referral Bonus for qualifying positionsInterborough is an EEO employer and provides equal employment opportunities to all employees and applicants for employment and prohibits discrimination and harassment of any type without regards to race, color, religion, sex, national origin, disability status, genetics, protected veterans' status, sexual orientation, gender identity or expression, or any other characteristic protected by federal, state and/or local law and encourage women, veterans and other minorities to apply.Interborough Developmental and Consultation Center is committee to Diversity, Equity, Inclusion and Belonging (DEIB) in the workplace.
