Seward Community Health Center is seeking a Temporary Full-time Clinic Nurse (RN or LPN) for a 3-6 month assignment at our small community health center located in beautiful Seward, Alaska. Anticipated dates of coverage are October 2024 - March 2025. The salary range for this position starts at $35-$50/hour depending on experience and licensure. In addition, SCHC will offer a $2,500 hiring bonus as well as a $2,500 completion bonus. No on-call hours required! Learn more about life in Seward, Alaska at Job Purpose Clinic Nurses provide direct clinical support to medical providers, clinic staff, and patients. This position supports patient care delivery by assisting with call-backs, prescription refills, referrals, patient education, prior authorizations, prioritizing need for appointment based on patient signs/symptoms, and by ensuring appropriate appointment scheduling. Essential Duties and Responsibilities Triage patient phone calls, requests and messages using the designated communication systems. Prioritize for specific action needed for provider intervention or contact, medication refills, provide for available appointment slots, etc. Handle and refer emergencies appropriately, including crisis intervention and triage. Facilitate provider efficiency and quality of care. Serve as first point of contact for questions from front desk personnel. Process medication refill requests. Assist with medication and immunization administration in clinic. Oversee patient flow, monitor waiting room, exam room availability, provider schedule and information and patient flow between the front office staff and clinical support staff. Ensure smooth flow of patient visits with minimum waiting time and high levels of patient satisfaction. Review provider schedules for appropriate scheduling times. Complete nurse visits. Assist in direct nursing care and education of patients as needed. Assist with procedures and medical assessment of patients as appropriate in conjunction with providers. Assist in escorting patients back to exam rooms if there is a backlog of patients waiting. Conducts a thorough chart review of scheduled patients for all providers in advance of the appointment to identify health maintenance and best practice deficiencies, orders POCT or labs to assist in meeting identified deficiencies in accordance with standing orders, assure referrals have been done and notes have been received, and reviews notes from outside offices to get a general idea of the plan of care. Tracks and conducts patient follow up calls for all patients seen in the clinic or the ER. Participates in morning huddle and other care team meetings. Assists patients in gaining access to services, coordination of care, oversight of their entire case, and linkage to appropriate services. Coordinates with providers and nursing team for Chronic Care Case Management patients. Assists primary care providers in developing individualized integrated care plans for patients, making referrals, and monitoring and evaluating care and referrals. Under general supervision, promotes patient awareness of health and wellness issues, plans, organizes, and conducts health education classes. Prepares and presents health education materials and encourages healthy behaviors that incorporate self-management goals and tasks with patients. Provides informational resources to patients on other possible services in the community and refer patients to the appropriate providers, to assist in the coordination of care. Works on Patient Educator referrals, and documents participation. Recruitment for workshops will be done as needed through promotional efforts and other venues as required. Help patients learn about their health and provide resources to manage their conditions. Help patients gain access to medical, social and educational services. Provide patients with credible, easy-to-understand health information through classes, workshops, conferences, and one-on-one patient meetings. Works closely with the Medical Director, and Social Worker as a key member of the SCHC Care Team. Works with the Outreach & Enrollment Coordinators and participates in community outreach events and presentations as needed. Performs other related duties as assigned. Qualifications Alaska LPN or RN license required. At least one year of primary care work experience required. Two years' experience in a family medicine clinic setting is highly desirable. Other combinations of education and experience may be considered. Skill in planning, organizing, implementing, evaluating and administering health education programs. Previous Electronic Health Record experience (especially Epic Ambulatory) a plus. Possess and maintain Basic Life Support (BLS) certification Current on tuberculosis screening Current on Hepatitis B immunization Ability to work with a diverse population in a kind, caring and compassionate manner and communicate effectively with patients from a variety of backgrounds. Ability to skillfully lead, empower, and build on patients' strengths in an effort to navigate patient self-management and healthy behavior changes. Ability to demonstrate commitment to the provision of care coordination services for the underserved. Excellent communication, organizational skills are required. Excellent computer skills required, especially familiarity with electronic health record systems (esp. Epic Ambulatory) and Microsoft Office (Word, Excel, PowerPoint, Publisher) a plus.
