JOB SUMMARY The KIC Clinical RN is responsible for the timely delivery of professional nursing care to patients of all ages and providing case management services for all eligible KIC patients which promotes quality and cost-effective health care outcomes working within established standards for case management practice and appropriate patient access. JOB DUTIES & RESPONSIBILITIES Coordinate and perform various nursing care procedures for patients of all ages, requiring professional knowledge and consideration of specific patient conditions and treatments, and the ability to work independently in completing assignments. In conjunction with the primary care provider, develops a plan of care from assessment data for individual patients to actively reach their medical goals which maximizes healthcare outcomes. Uses problem-solving, critical thinking and anticipatory skills in meeting the needs of the patient and the provider. Patient needs should be addressed in a prompt efficient manner Performs staff nursing duties in the various nursing roles in the clinic. Duties include assessment, documentation, education and administering appropriate medications and/or therapies. Document observations, nursing interventions, health teaching, therapeutic measures, and the coordinated activities between nursing and other professional disciplines. Evaluates to what extent nursing interventions have met the health care goals the patient and revises the plan of care accordingly. Interdisciplinary approach to facilitate and coordinate all patient referrals. Tracks progress of referrals and outcomes of care, providing periodic status report to providers on outstanding referrals. Maintains annual competencies including safety and infection control requirements. Performs duties as assigned in a responsible, respectful, and caring manner to promote effective nursing staff relationships. Maintains a clean and safe working environment including adequate supplies and upkeep of unit and equipment, reporting any defective or missing equipment and safety hazards. Adheres to infection control policy and procedures. Serves as primary clinical contact regarding patient care and in care coordination for the patients. Proactively identifies delays and obstacles to care and works to remedy individual cases and improve the process. Works collaboratively by building bridges and creating rapport with team members within departments and across the organization. Participates in department and clinic team and improvement efforts. Performs other duties as assigned, including special projects deemed necessary to provide comprehensive health services. RN I MINIMUM QUALIFICATIONS (Education & Experience) Associate or Diploma in Science of Nursing from an accredited school of nursing. Licensed in the State of Alaska as a Registered Nurse (RN). Basic Life Support (BLS) certification required. PREFERRED QUALIFICATIONS (Education & Experience) Bachelor of Science in Nursing (BSN) preferred One year of ambulatory or other nursing experience preferred. Experience in community clinic preferred and a strong commitment to multi-disciplinary teamwork. RN II MINIMUM QUALIFICATIONS (Education & Experience) Bachelor of Science in Nursing (BSN) from an accredited school of nursing. Three years of experience as a Registered Nurse Licensed in the State of Alaska as a Registered Nurse (RN). Basic Life Support (BLS) certification required. On a year-for-year basis, a combination of relevant experience, education, and training may substitute for education and experience requirements. PREFERRED QUALIFICATIONS (Education & Experience) Three years ambulatory nursing experience ACLS/PALs certification Experience in community clinic preferred and a strong commitment to multi-disciplinary teamwork. NECESSARY KNOWLEDGE, SKILLS, & ABILITIES The nursing process and the ability to apply this knowledge in the working environment. Safety and application of infection prevention and control principles. Assessment, anticipation of needs, and data collection. Strong understanding and sensitivity to American Indian/Alaska Native (AI/AN) Tribal culture and lifestyle, working effectively with AI/AN populations. Effective oral and written communication skills with sensitivity to the lifestyles of targeted participants. Skills in operating personal computers using a variety of computer software. Perform job using theory-based nursing practice model, evidence-based practices, and shared governance in a multi-disciplinary team. Ability to establish and maintain cooperative working relationships with management, coworkers, and outside contacts. Ability to maintain strict confidentiality and comply with all applicable federal and tribal privacy laws and KIC policy (HIPAA, HITECH, etc.). Ability to learn and apply program/agency operating policies and procedures. Ability to manage multiple priorities and tasks concurrently and meet deadlines. Ability to work independently as well as with teams.
