There are four (4) Assignment Levels within this class of positions; all personnel perform related work. The following are typical assignments within this class of positions.General tasks and responsibilities will include:• Provides appropriate nutritional care for patients in various outpatient practices including but not limited toAdult Medicine, Pediatrics, Geriatrics, Oncology, Women's Health Center, Immunology, Dialysis Center and group nutrition education classes. May relieve inpatient areasunits as assigned.• Interviews patients for nutritional history. Completes timely documentation in patient's electronic medical record such as nutritional assessments, follow-up notes and patient goals.• Provides nutritional counseling, diet instructions and fooddrug interaction counseling.• Interprets pertinent laboratory and other diagnostic data to determine appropriate care plan.• Utilizes and develops as needed appropriate nutrition education materials (age specific, culturally sensitive, low health literacy, and condition specific).• Plans nutritionhealth programs, implements, and evaluates nutrition education in different settings.• Participates in interdisciplinary roundingdischarge planningmeetings and other appropriate patient caremeetings. Consults with physicians and other health care faculty members as necessary.• Conducts rounds at meal time to observe patient food intake and provides them with nutrition information, which will enable them to make nutritionally balanced food choices; submits meal round reports in a timely manner.• Teaches nutrition classes as needed to patients, employees, students and health care team members.• Reports problems to immediate supervisor and recommends ways to improve nutritional services.Establishes rapport with all members of the health care team and within the department in order to assuregood public relations and quality nutritional care to patients.• Attends dietitians' meetings, nutrition committee meetings and weekly departmental huddles as scheduled. Participates in the development and implementation of quality assurance activities including Productivity Reports, as assigned. Completes accurate Productivity Reports at end of shift daily.• Assists in orientation, training, supervision and evaluation of nutrition students, Dietitian I and 11.• Maintains continuing education in nutrition, clinical dietetics, food service management and related areas as appropriate in order to update current knowledge and skills in the practice of dietetics and performance of assigned.Minimum QualificationsBenefitsQualification Requirements:For appointment to Assignment Level I1. Baccalaureate degree from an accredited college or university, with a major in Dietetics or Foods andNutrition in a program approved by the American Dietetic Association (ADA); or,2. Registration or eligibility for registration by the Commission on Dietetic Registration (CDR) as a RegisteredDietitian or New York State Certification as a Dietitian-Nutritionist.For appointment to Assignment Level II1. Registration or eligibility for registration by the CDR as a Registered Dietitian or New York State Certificationas a Dietitian-Nutritionist.For appointment to Assignment Levels III and IV1.
