Helping U Homecare - Helping U Home Care (HUHC) is looking for an RN Home Care Nursing Supervisor to join our exceptional interdisciplinary team of home health care providers. Do you have experience working at a New York State Licensed Home Care Agency providing Supervisory and Assessment visits?  We are looking for compassionate, dedicated, and well-rounded professionals who can uphold our standards and culture. Qualifications: Current and valid NYS RN License Minimum 1 year of home care experience doing Supervisory Visits and Assessments (Preferred) Ability to accurately assess patients' health care status and psychosocial needs Communicate effectively, both verbally and in writing with physicians, patients and caregivers Experienced with Microsoft Office and HHA Exchange a plus Reliable means of transportation to conduct home visits in at least 2 NYC Boroughs Responsibilities: Conduct in-person initial patient assessments, re-assessments, quality assurance and supervisory visits of HUHC's field staff (paraprofessional caregivers and nurses) Assist with developing, implementing and revising patient's Plan of Care Ensure that all HUHC caregivers adhere to the Plan of Care and Physicians Orders   Provide assistance to DPS and Coordination in recommending ideal caregivers for each patient, taking into account clinical and psychosocial needs of the patient Communicate effectively with HUHC team members to ensure each patient is receiving the highest quality home care from HUHC's caregivers Additional duties as assigned Benefits:  Health Insurance  Dental Insurance  Paid time off  Flexible Scheduling  401 (k)  Growth opportunities Job Type: Full-time Salary: $80,000.00 - $100,000.00 per year Work Location: Manhattan office but on the road at least 50% of the time
