Job Details Job Location Castle Gardens (West Harlem) - New York, NY Salary Range $45,000.00 - $55,000.00 Hourly Travel Percentage Negligible Job Shift Day Shift Description Title :Case Manager Unit: Scattered Site Housing Reports to : Program Supervisor Status: Full Time, Regular, Non Exempt Salary Range: $45,000 - $55,000 Location: 625 W 140th Street New York, NY Days/Hours: Monday - Friday 9:00am -5:00pm (flexibility required) Position Summary: The Case Manager will screen, assess, counsel and link homeless, formerly incarcerated participants with histories of mental illness, substance use and/or HIV/AIDS to needed transitional and permanent housing. Case Manager will provide ongoing supportive services to assist participant to maintain housing. Essential Duties and Responsibilities: Conduct intake interviews and needs assessments on newly released individuals and participants eligible for housing services; Create individualized treatment plans for each participant. Document all participant services, including successful and unsuccessful outreach attempts; Services must be documented in a timely manner to ensure that the participant records remain current; Conduct crisis intervention with participants, as needed; Develop and maintain appropriate housing stock for our participants; Assess participants need for temporary rental subsidy and furniture/moving allowance; Collaborate with Housing Preservation and Development (HPD), Department of Homeless services (DHS), and Human Resources Administration (HRA) to assure participants receive entitlements in a timely manner; Assist participants in obtaining all necessary documentation to facilitate benefits application plete and submit participants' application to appropriate benefits provider agency (e.g., HRA, Social Security); Act as a liaison to representatives of benefits administrations; Act as liaison between participant and landlord to mediate any landlord-tenant issues and Provide ongoing counseling and supportive services needed to achieve goals stated in participant treatment plan. Conduct home visits, hospital visits and other community-based visits as needed. Conduct annual apartment inspections to insure a safe, secure living environment. Work with Property Management to ensure that needed repairs are facilitated. Escort participants to appointments when needed. Conduct case conferences with community based service providers when needed. Provide coverage for participants outside of caseload when needed. Qualifications Qualifications: Education- Associate's Degree in Human Services or/and five years of experience working with special population; a High School Diploma and eight years of experience working with special population or a bachelor's degree in human services is preferred with at least 2 years of experience with the target population. CASAC or CASAC-T certification with 2 years' experience working with population is preferred. Work Experience- Candidate must have the ability to oversee a maximum case load of 15 addition, must be able to provide a collaborative assessment of needs, create and implement service plan, monitor progress and evaluate services provided to the individual participant. Computer Skills- Proficient in Microsoft Excel, Word, and Outlook with a demonstrated experience in accounting software and working knowledge of PowerPoint. Communication Skills -Candidate will possess the ability to have excellent writing, editing and verbal communication skills. General Work Behaviors/Skills - Excellent interpersonal skills with ability to interact culturally, linguistically, and diplomatically with diverse internal and external individuals.
