Serving residents in a very active community full of fun, family, and opportunities to serve.) What we offerCompetitive salary $58000.00- 69000.00Benefits include medical, dental, vision, generous Paid Time Off program, holidays, 401k and more!!!A growing company with opportunities for advancementCompany sponsored training, tuition reimbursement, and other learning opportunitiesJob DescriptionThe Director of Social Services is responsible for planning, developing, organizing, implementing and directing the SNF Health & Wellness in accordance with current existing federal, state and local regulations, as well as Stellar Senior Living established policies and procedures, to ensure medically related social services are provided to all residents in the facility. ResponsibilitiesPlan, organize, develop the overall operation of the Social Services department to ensure all medically related emotional and social needs of patients are metEnsures the quality of social services rendered by the facility at all timesParticipate in the admission, discharge, MDS coordination, etc for social servicesRefers patients and families to appropriate community resources as necessaryQualificationsMust be currently registered/ licensed in the StateBachelor's Degree in Social Work or equivalent At least two years of experience in Long Term Care preferred.Demonstrated knowledge or rules, regulations and guidelines that govern nursing care facilities.  If you are the right candidate, then we definitely want to hear from you!
