As a Licensed Professional Nurse/Licensed Practical Nurse you will:Provide intermittent Skilled Nursing services including assessment, evaluation, procedures, teaching and training activities, including activities of daily living as outlined in the patient Plan of CareSubmit completed skilled nursing visit notes as requested by Clinical SupervisorParticipate in discharge planning process per Medicare CriteriaAttend staff meetings, team conferences and educational in-services as requiredBe responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care  QualificationsQualifications A current unencumbered State Professional Nurse LicenseActive CPR CertificationMust maintain a valid driver's license, reliable transportation, and good driving recordThe ability to make sound professional clinical judgmentThe ability to assess and document patient needs and meet those needsProficiency in Microsoft Office Pay TransparencyIndividual compensation packages are based on various factors unique to each candidate, including skill set, experience, qualifications, and other job-related considerations.
