With 142 independent living, assisted living, memory care apartments and 64 long-term and transitional care suites, our Brooklyn Park campus is a vibrant, established community where it's easy to establish positive connections.ResponsibilitiesThe RAI Coordinator is responsible for accurately completing the RAI process/Resident Interviews for assigned residents, attending resident care conferences, reviewing charts and admission information and reporting areas of concern to the appropriate Clinical Coordinator, and attending discharge meetings when directed. Qualifications:Education: Training as a Registered Nurse. Certification in Gerontology preferred.Experience: Licensed in Minnesota as a Registered Nurse, in good standing. Minimum three years long-term care experience. Experience with PPS, 3rd Party Payors, MDS process, Case Mix, Medicare and Care Planning.Special Knowledge, Skills and Abilities:Demonstrated knowledge of State and Federal regulations, including Resident Assessment Instrument.Demonstrated extensive knowledge of Case Mix.Demonstrated extensive knowledge of Medicare, HMO's and Managed Care.Good written and verbal communication skills.Demonstrates knowledge in ICD9/10 Diagnosis CodingFlexible with time management and prioritization skills.Ability to maintain an effective working relationship with nursing staff and other departments.Demonstrated knowledge of software (Micro Soft, ROX, Facility Software, Excel.)Ability to work with others in identifying and improving quality of care issues and solutions.High level of respect and commitment to working independently and with others.
