You willplay an integral role by supporting individuals and their familiesin accessing support services within their localcommunity.Objectivesfor thisrole:Provide detailed information, advice and guidance usingmotivational interview techniques, that will be delivered inperson, by telephone andemail. Workclosely with clients to develop a Personalised Care and SupportPlan (PCSP) understanding their needs, the desired outcome and thenecessary interventions to providesupport. Signpostand make appropriate referrals to voluntary sector services,community initiatives, and relevant public sector services toachieve agreed actions andoutcomes. Offershort-term support to clients, fostering independence, confidence,and skilldevelopment Bepart of a multidisciplinary team focused on supporting individualswhose housing situation impacts their health and well-beingpositively, providing practical and holisticassistance.Whatwere lookingfor:Clear communicator and ability to engage with individuals fromdiversebackgrounds.Ability to work collaboratively in a multidisciplinary teamenvironment.Willingness to adapt to changing organisational needs andpriorities.Knowledge of voluntary and community services in Norfolk and how tosupport people to accessthem DriversLicence and Car isessential.Afull training program will be given.
