Job Description

The Children’s Continuing Care Team provides commissioned care in the home and school setting for children with complex health needs.The Long Term Ventilation Team provides packages of care in the home for children who require ventilation to assist with their breathing.Swanbourne Gardens is a 4-bedded respite unit that provides overnight respite for children with complex health needs.We strive to provide the children we support with a high standard of care. As a Band 5 nurse working within the team, you would be responsible for the assessment, planning, implementation, and evaluation of patient care.You would be at the forefront of teaching and assessing the competencies required for the staff to deliver day-to-day care for children with complex health needs, including interventions such as care and management of tracheostomies, gastrostomies, and ventilation support. In addition, you would be expected to provide supervision for junior staff.As a Band 5 nurse, we expect you to role model the trust's value and behaviour framework while working within the team.Robust training and regular competency reviews will be provided to ensure care is delivered safely and in line with all policies and guidelines. The successful candidate will be based at Swanbourne Gardens but will provide care in other community settings such as the child or young person's home or school.You must have a full driving licence and access to a vehicle for work purposes.Responsibilities include:Assessing, planning, implementing, and evaluating child and family-centred care. Teaching and assessing the competencies of the Health Care Support Workers. Undertaking a range of duties involving direct care, following care plans agreed with parents and named professionals while working in various settings. Being aware of and participating in meeting the psychological, social, physical, and spiritual needs of the children and young people. Maintaining accurate records of all care given, omissions, and ensuring effective communication with parents and named professionals regarding any changes in a child's condition or situation. Promoting professionalism at all times.We are looking for staff who are flexible, as we work around the needs of the families and children, which requires staff who can adjust their hours to start early, finish late, and work overnight.We are seeking a Band 5 Paediatric Nurse. The post holder will have post-registration experience, preferably working with children with complex health needs, either in a hospital or community setting.Full training and supervision will be available from our existing senior team, but any previous experience would be desirable.We are a very small but supportive team that can offer guidance in developing new skills and adapting to a new environment.We hold a unique position in the Stockport community as a healthcare provider and are one of its largest employers.We believe that the best organisations reflect the communities they serve.
