Job Description

We are looking to appoint a passionate and committed Practice Nurse for our friendly training practice based in Lytham St Annes. The applicant must be RGN registered and have relevant General Practice Nursing experience. With a teamwork focus you will help us in developing primary care services to the practice population and the Network. Main duties of the jobMain Duties Chronic disease & Long term disease management Cervical Cytology Screening Childhood Immunisation Clinic About usWith a list size of around 12,500 patients under the GMS contract, you will be working alongside our clinical team of GPs, Pharmacists, Physician associate, Phlebotomists, HCAS, and nurse Practitioners. Job responsibilitiesJOB TITLE:Practice Nurse To assist medical personnel in the care of patients to include treatment, preventative care, screening and patient education.MAIN DUTIES AND RESPONSIBILITIES: To provide assessment, screening treatment services and health education advice. Provide nursing treatments to patients in participation with general practitioners or independently to agreed protocols. Flu Clinics & Health Promotions: Provide general and specific health screenings to the practice patients (within agreed protocols) with referral to general practitioners as necessary. Advise patients on general health care and minor ailments with referral to GPs as necessary. Treatment room supplies and equipment: Work in participation with the nursing team to ensure the maintenance of equipment and stock levels relating to patient care. Pathological specimens and investigatory procedures: Undertake the collection of pathological specimens including intravenous blood samples, swabs, smears etc. Perform any investigatory procedures requested by the GPs. Administrative and professional responsibilities: Support and lead the nursing team to ensure the best service and standard of care to patients. Participate in the administrative and professional responsibilities of the practice team. Ensure accurate notes of all consultations and treatments are recorded in the patients notes. Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice. Ensure collection and maintenance of statistical information required for regular and ad hoc reports. Attend and participate in all practice meetings as required. Research projects: Co-operate and participate as required in any research projects within the practice. Education and training of students and practice staff: Participate in the education and training of students of all disciplines and the introduction of all members of practice staff. Maintain effective liaison with other agencies and staff concerned with patient care and with all other disciplines within the practice, with appropriate regard to confidentiality. Professional development: Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development. Special requirements of the postRegistered General Nurse. Membership of a professional body. To work in accordance with the NMC Code of Conduct. Ability to self-motivate, organise and prioritise workloads.
