Mid Cheshire Hospitals NHS Foundation TrustWe are looking for an experienced Band 3 HCA to cover 4 night shifts per week within the District Nursing Out Of Hours Service. The role includes caring for patients who are approaching the end of life. It involves working on a one-to-one basis, spending a full shift with one patient in their own home. You will be working without close supervision, and will therefore need to use your own initiative while following the patient's care plan. You must also be passionate about making a difference to people's lives, helping them to live as well as possible and to die with dignity. As well as being able to work independently, you will possess good time management and organizational skills and be expected to be innovative and flexible to work in other areas if required to meet service needs. We will expect you to have a talent for forging strong empathetic relationships with a variety of people while providing palliative care. We will of course provide excellent ongoing training and support. Experience of caring for people at the end of life is essential. Main duties of the jobYour role will include delivering a range of nursing interventions to patients in their own homes. These may include monitoring symptoms, undertaking simple dressing and continence care, and keeping detailed records. You will also help patients to meet their hygiene and dietary needs. You will be adaptable, reliable, and approachable and will be expected to support your peers, the team leader, and the organization in promoting the Trust's Values and Behaviors. Please note while your main base will be at Crewe, you will be required to work across the footprint of Central Cheshire Integrated Care Partnership. Job descriptionJob responsibilitiesFunction as an effective member of the multi-disciplinary team. Maintain clear, concise, and legible documentation adhering to prescribed standards and protocols. Use IT systems for relevant tasks e.g., accessing patient records & results, data collections, and inputting data. Obtain consent in accordance with Trust Consent Policy. Report accidents/incidents on clinical incident reporting system of patients or self which occur whilst on duty. Maintain a patient-oriented approach towards care delivery, promoting a rehabilitative and self-care approach. Undertake phlebotomy duties as delegated and overseen by the registered nurse, working within the policies and procedures of the Trust. Undertake clinical tasks as delegated by the Community Nurse team and within the sphere of competency. Assist Community Nursing Team in management of patients with Long Term Conditions and in End of Life. Recognise the need for referral and liaise with other health care and multi-agency workers as necessary. Participate in the induction and orientation programmes for all grades of staff who require community experience as arranged by the Team Leader. Learn and maintain an up-to-date knowledge of the blood tests performed and select the appropriate sample tubes for the analysis requested, ensuring that they are correctly labelled at the time of sampling. Maintain the phlebotomy stocks, keeping them clean and in good order. Follow Trust policy for any needle stick incidents and ensure reported accurately on Ulysses system. Dispose of all syringes, needles, etc. in accordance with health and safety procedures. Person SpecificationQualificationsEssentialNVQ Level 3 in health care or equivalent experience. DesirableWillingness to undertake NVQ level 3 or equivalent. Knowledge and ExperienceEssentialFollowing training, able to work without direct supervision, and able to recognise own limitations. Demonstrates the ability to follow guidelines and procedures within agreed boundaries. Experience of working in a healthcare environment. DesirableWorking within a community setting.
