They are looking for a new member for their Hospital Team. The position is required to facilitate hospital discharge and implementation of new operating model that brings discharge functions into the community SW teams. There is also a need to work with colleagues and partners in a range of organisations to develop the appropriate service and support to meet the needs and outcomes of vulnerable residents, as well as ensure safeguarding procedures are adhered to and implemented accordingly.
