The service currently operates 7 days a week between the core hours of 0800-1830. Shift patterns available are varied and can be discussed at interview with a flexible approach. Posts are available in various South West district nursing teams. Main duties of the jobThe responsibilities of the role include, but are not limited to: Provide holistic assessment, planning, delivery, and evaluation of care. Deliver skilled nursing care in patients’ homes and community settings. Supervise and support colleagues, including students and junior staff. Identify and respond to early signs of patient deterioration. Person specificationQualificationsRegistered General Nurse (Adult). Educated to degree level or equivalent. Knowledge and ExperienceKnowledge around long term conditions and their management. Evidence of ongoing professional development. Ability to supervise and support a range of individuals including team members, formal and informal carers. Demonstrates leadership skills. Ability to plan and organise own and others work. Previous experience of implementation of change. Previous Community Services experience. Technical expertise/competenceDemonstrates an understanding of local and national issues influencing the delivery and development of health care. Demonstrates a knowledge and understanding of evidence based care. Demonstrates an understanding of the Clinical Governance requirements. European Computer Driving Licence or equivalent.
