Job summaryAn opportunity has arisen for a highly skilled, enthusiastic and motivated Physiotherapist to join the management team of Integrated Care across Greater Nottingham.FDIDT are an in-reach multidisciplinary team made up of AHPs, Nurses and Assistant Practitioners. This Nottingham CityCare Partnership service provides innovative, successful and patient focused outcomes for those that present in the emergency and urgent pre admission areas at Nottingham University Hospitals QMC site.Your role within the busy and dynamic team will be to provide a comprehensive holistic assessment of need with the aim of preventing avoidable hospital admissions.The successful candidate will be able to work within the MDT as an autonomous practitioner, undertake comprehensive assessments, and complete profession specific assessment and evaluation as required, to identify the appropriate discharge plans for adults with a wide variety of complex physical and mental health needs.The Teams Mission Statement is to provide a gold standard assessment and review of patient needs that allows ongoing treatment to be provided by the right service, at the right time, in the right place without delay or duplication. This is underpinned by our organisational aim of building healthier communities and being committed to the development and future of an Integrated Care System across Nottinghamshire through active membership of the local integrated care provider partnership.Main duties of the jobTo work as an autonomous practitioner, you will undertake comprehensive assessments to identify the appropriate service for the clients referred. To carry out profession specific assessment and evaluation as required. To work within the MDT to plan and develop discharge plans for adults with a wide variety of complex physical and mental health needs. To be responsible for continuing the on-going development of high standards of clinical practice with evidence-based focus within the service. To work, as required within the integrated care and Front Door Integrated Discharge services, reacting to emergency health and social care referrals with the aim of preventing admission to hospital. You will be required to work primarily with the Queens Medical Centre but with potential to work across different bases and community settings which may include clients own home, residential, nursing homes or hospital. You may be responsible for a designated caseload of clients. You will be required to participate actively in City Cares training requirements. To undertake the comprehensive assessment and evaluation of the patients Physical and Mental health needs to develop individualised discharge plans/rehabilitation/reablement goals. To undertake specific professional assessments to ensure the best outcomes for clients. To use clinical reasoning and a wide range of skills and knowledge to devise specialised programmes of care. To be responsible for coordinating the safe and timely discharge process from a variety of settings, including hospital and community beds. To undertake comprehensive evidence-based risk assessments developing risk management plans. To provide specialist advice, education, and mentorship to other members of the team and colleagues regarding the clinical management of clients. To actively engage with families and carers in order to ensure the best outcome for the service user. To demonstrate flexibility, prioritise effectively and time manage own clinical workload. Be able to delegate appropriately. To lead in regular team meetings and review meetings regarding client caseload. To keep accurate and timely service user records to ensure continuity of care. To use information technology daily and participate in identified data collection and statistics and maintain clear, concise, and timely records in line with the service policy. To participate in the Lone Worker Tracking procedure as applies to the team. To act as duty clinician for the process of referrals. To undertake any other duties which may be reasonably required.Education and Leadership To be involved in formal appraisal systems, supervision, and be active in identifying aims and objectives for personal development in line with departmental policy. Also, to keep an active up to date CPD portfolio. To participate in the education of colleagues, junior staff, students, and multi-professional teams using excellent mentoring and presentation skills. To assist in the training and development of others (new members of staff, students, and visitors to the service). To keep up to date with IT skills in line with the modernisation of data collection records. To participate in appropriate in-service training and individual learning with specific and measurable outcomes.Person SpecificationExperienceEssential Experience of working within the UK health system. Relevant experience of rehabilitation with adults. Awareness and understanding of community services. Experience of working in a multi-disciplinary team. Experience of supervising junior, unqualified staff, and students. Desirable Experience working within intermediate care services. Experience of working with clients with mental health needs. Minimum of 2 years post graduate experience. Community experience.
