The post holder will support Neighborhood teams to deliver locally driven high quality care, responsive to patient and carers needs. This includes education of other health care professionals, training, support and advice for the multidisciplinary team.The focus of the multidisciplinary approach will be to support patients and carers to live well, prevent inappropriate hospital admission and meet their preferred priorities for care.Main duties of the jobAn exciting opportunity has now arisen within the City Wide Manchester Macmillan Palliative Support Service for a Clinical Nurse Specialist in Palliative Care. The post holder will work as part of the Macmillan Specialist multi-disciplinary team comprising of Medics, Dietitian, Occupational Therapist, Physiotherapist, Nurses, Speech and Language Therapist, Assistant Practitioners, Admin support and Volunteer Coordinators. The team aims to provide effective support palliative care and/or rehabilitation to patients with a life limiting condition requiring palliative intervention.The post holder will provide supportive palliative care to enhance the quality of life for patients/carers with any life limiting illness requiring palliative care, including complex physical, psychological and emotional needs.
