This is a 22-hour per week role with a fixed 12-month contract. This role would be suitable for an existing diabetes specialist nurse or a community or practice nurse with an interest in managing diabetes.Main Duties of the JobHelp support and manage people with diabetes referred to the service within a defined population as well as provide diabetes advice and guidance to other health care professionals as required.Use clinical skills for assessing and treating patients with diabetes.Provide a point of contact for the patient population offering specialist advice and referral routes.Communicate with service users referred into the service with respect and empathy, using a person-centred approach and motivational interviewing skills to promote self-care and self-efficacy.Monitor and review people with Type 1 or Type 2 diabetes in the community via virtual, telephone, face-to-face clinics or group education regarding symptom control, prescription, and titration of medication adhering to local guidelines.Deliver diabetes care in a clinic setting: face to face and remotely via video, telephone, and email following clinical guidelines and triage guidance.#J-18808-Ljbffr
