The successful candidate will play a crucial role in organising and providing a comprehensive pharmacy service across all renal and urology wards, outpatient services, and dialysis units within our health system. They will support to ensure the day-to-day provision of an efficient pharmacy service tailored to the needs of renal patients, ensuring high standards of care.In this role, the post holder will be required to demonstrate comprehensive skills in medication management, including medication history taking, near-patient dispensing, and facilitating discharge planning related to medications and counselling. They will also have the opportunity to monitor and enhance the technical elements of the pharmaceutical service for renal patients, aligning with the objectives set by the Lead Pharmacist for Renal Services. The specialist workload will comprise patients within the nephrology, dialysis and renal transplantation services, and the postholder would be expected to be an integral part of the multidisciplinary team.Main duties of the jobThe post holder will be responsible for organising and providing a comprehensive pharmacy service to all renal wards, outpatient clinics and dialysis units across North Central London. This role will support medication optimisation for dialysis patients and help bridge the gap between primary and secondary care.In addition, the post holder will need to demonstrate strong medicines management skills, including medication history taking, original pack dispensing at ward level and facilitating discharge with regard to medications and counselling.Finally, responsibilities will also include the oversight, development and delivery of pharmaceutical services for renal patients in accordance with the goals established by the Lead Pharmacist for Renal Services.
