Join us as we continue to grow and innovate in the mental health space!Key Responsibilities:Key ResponsibilitiesAssessment and Evaluation- Conduct thorough assessments of patients' social and emotional needs, including their family, community, and environmental factors. Develop detailed psychosocial assessments to inform treatment planning.Care Planning and Coordination- Collaborate with the multidisciplinary team to develop and implement individualized care plans. Ensure that social work interventions are integrated into the overall treatment plan.Counselling and Support- Provide individual and group counselling to patients and their families. Address issues such as coping with mental illness, family dynamics, and social adjustment.Crisis Intervention- Respond to crises and provide immediate support to patients and families in distress. Develop and implement crisis intervention strategies as needed.Resource Linkage- Assist patients and families in accessing community resources and support services, including financial assistance, housing, and vocational training. Provide information and referrals to appropriate services.Advocacy- Advocate for patients' rights and needs within the healthcare system and the broader community. Work to reduce stigma and barriers to mental health care.Family Support and Education- Educate families about mental health conditions, treatment options, and available resources. Support families in understanding and managing their loved one's mental health needs.Home and Community Visits- Conduct home and community visits to evaluate patients' living conditions, social support systems, and community resources. Use these visits to provide therapeutic interventions and support, ensuring that care plans are relevant and effectively implemented in the patient's environment.Collaboration- Work closely with psychiatrists, psychologists, nurses, and other healthcare professionals to ensure a holistic approach to patient care. Participate in case conferences and treatment planning meetings.Documentation and Reporting- Maintain accurate and up-to-date patient records, including assessments, treatment plans, and progress notes. Ensure documentation complies with legal and professional standards.Program Development- Contribute to the development and implementation of social work programs and interventions aimed at improving patient outcomes and enhancing community support.Professional Development- Engage in continuous professional development to stay updated on best practices and emerging trends in psychiatric social work.Qualification and Skills: Educational Background: Master's degree in Social Work (MSW) with a specialization in psychiatric or clinical social work from a recognized institution.Certification: Registration with the relevant state or national social work regulatory body (e.g., The Indian Association of Social Workers).Experience: 1-5 years of experience. Previous experience in a mental health setting is preferred. Demonstrated experience in providing counselling, crisis intervention, and social work services.Skills: Strong interpersonal and communication skills, with the ability to build rapport with patients and families.
