New Value Solutions, a national IT consulting company is seeking a part-time ICF Medical Subspecialist to assist with development of an ICF-based care coordination tool for implementation of the ICF into service delivery at a centre for health complexity. The care coordination tool will support staff with a structured approach and common language to enable delivery of an equitable, individualized, family-centered approach. Further, the ICF tool will prioritize function, and child and family voice in the delivery of the care coordination service. Responsibilities: Work collaboratively with the project''s clinical planning team to accomplish five deliverables: Support the completion of version 2 of the minimal function set for the population (i.e., children and youth living with health complexity). Lead the development of an ICF minimal functional dataset toolkit or dictionary (i.e., a set of standardized sources and approaches to how ICF components within the minimal functional data set will be measured). Support the development of an ICF care coordination tool to be used within the context of the new centres care coordination service. Lead the development of a list of the next step tools to be developed for implementation in clinical service at the new centre. Should time permit, lead the writing of a manuscript for publication focused on ICF work to date, to be co-authored with project team members. Requirements: A medical subspecialist in pediatrics or developmental pediatrics, or equivalent. Knowledge and experience with patient and family centered care program development and evaluation. Significant previous experience supporting implementation of the ICF into pediatric clinical care initiatives for subspecialty pediatric populations. Experience guiding the development of minimal functional set(s) for subspecialty pediatric population(s). Strong systems-level understanding of pediatric health care in Canada. The ability to work collaboratively with a project team made of diverse partners (with clinical and non-clinical background), as well as a wide-range of external partners, including partners with lived-experience. The demonstrated ability to respond flexibly to changing goals, needs and deliverables. Strong, verbal and written communication.
