Your Opportunity: As a Primary Care Clinical Pharmacist, you will be part of the team in the fast-paced practices of family physicians. You will work collaboratively with other health professionals to provide comprehensive, accessible primary health care, honouring patient choice in how best care will be delivered (in-person, telephone, video). Pharmacists in the Hamilton Family Health Team (HFHT) aim to improve drug therapy outcomes using a collaborative care model that integrates pharmacists into the primary care team. As a pharmacist working in this Family Health Team, you will: Integrate into various family medicine group practices by collaborating with physicians, other health care professionals and patients to promote optimal, patient centered care Provide individual patient assessments to identify, prevent, and resolve drug-related problems Provide pharmaceutical consultation, pharmacotherapy monitoring, education/teaching and drug information Support health promotion, disease prevention and chronic disease management Develop the practice based on the needs of the FHT To be eligible, you must: Have a Bachelor of Science degree in pharmacy or PharmD from a recognized university Be registered and in good standing in with the Ontario College of Pharmacists (Part A) Have completed a community or hospital residency or have equivalent clinical experience in either setting* Be able to work in an interdisciplinary team environment Be able to prioritize and be flexible in a very active work environment Demonstrate expertise in practicing pharmaceutical care, problem solving and critical thinking *Additional training or credentialing in pharmacotherapy, evidence-based medicine or pharmaceutical care is considered an asset (e.g. Board of Pharmacy Specialties, Certified Diabetes Educator, ADAPTTEACH) Why join the HFHT?
