The Neighbourhood Group (TNG) is a multi-service agency that includes Central Neighbourhood House, Neighbourhood Link Support Services and St. Stephen’s Community HouseJOB OPPORTUNITY Job#TNG2024-166POSITION TITLE: PSW SupervisorDEPARTMENT: Independent LivingSTATUS: Contract Full-Time, non-bargaining unit# HRS / WEEK: 21CONTRACT DATE: January 20, 2025 to April 20, 2025REPORT TO: Senior Manager, Client Care ServicesANNUAL SALARY: $60,640- $68,228GENERAL DESCRIPTION: Reporting directly to the Senior Manager, Client Care Services and working out of the 688 Coxwell office, TNG is currently seeking a PSW Supervisor. The incumbent will be responsible for supervising and coordinating Home Support Services and/or Personal Support Services in a manner that is responsive to clients’ needs (e.g. short and long term, respite, and palliative care). The position provides essential services, and therefore may be occasionally required to report to work during emergencies such as storms, power outages, or transit strikes, and beyond regular working hours.MAJOR DUTIES: Respond to referrals for service in a timely manner. Work with the PSW Supervisor to understand the profile of each referral for service. Assess client needs according to the timeframe established by contractual agreements, formulates the plan of care, and assesses progress towards outcomes.Maintain up to date electronic charting in client’s file. Collaborate with the PSW Supervisor to guide a team of PSWs and ensure compliance with TNG policies and procedures and contractual obligations of providing care in a safe and effective manner within the required timeline.Oversee the ordering and MOH reporting requirements of essential PPE to maintain a 3 to 4-month supply of personal protective equipment (PPE) for self and PSWs. Assess quality of PPE. Ensure continuity and consistency of care by working closely and being in constant communication with the PSW Supervisors. Follow proper protocol around conducting new client assessment visits, PSW field supervisory visits, quality assurance visits, re-assessment visits etc. Provide on-going case management, including service planning and monitoring client goals/outcomes.Prioritize safety of clients and PSWs and work within a risk management framework for client care.Connect clients and/or caregivers to other community programs and services as required. Promote TNG services such as Meals on Wheels and Adult Day Programs.Communicate and collaborate regularly with PSW Supervisors and office team to provide and receive information about client, family, caregivers and PSWs, referrals for other services, etc.Work collaboratively with multidisciplinary teams in a community setting.Discuss with staff any recommendations to alter the client’s service plan when a change in client status is identified by TNG, the Funder or client.Participate in orientation sessions and the delivery of in-service clinical and soft skill training and conducting meetings with community partners. Delegate procedures and/or provide training to PSWs.Provide crisis intervention support for clients, caregivers, and PSWs as required.Identify early signs of abuse and neglect and work in accordance with Duty to Report. Ensure PSWs follow proper reporting protocol. Prepare information to assist with performance appraisals and implementing progressive disciplinary action of PSWs.Assist with the integration of client satisfaction initiatives, ensuring that all concerns and complaints are satisfactorily addressed and documented in a timely manner.Ensure that all client/caregiver concerns and complaints are addressed and documented in a timely manner.Assist other PSW Supervisors with interviews of prospective PSWs.Other related duties as required.REQUIREMENTS & QUALIFICATIONS:Registered Nurse (RN) or Registered Practical Nurse (R.P.N.) Must have current registration with College of Nurses of Ontario and be in good standing.Mask Fit Test Training Certification preferred.Gerontology designation or equivalent experience. A valid Ontario’s Driver’s License is preferred.Minimum 3 years related experience and progressive leadership responsibilities, preferably in a community setting working with unionized staff. Progressively responsible work history including all aspects of supervision of staff including coaching, mentoring, performance management, training, on boarding and health/safetyExperience providing end to end support to new clients including conducting new client assessment visits, field supervisory visits, quality assurance visits and re-assessment visits. Knowledge of the principles, best practice, and methods of community nursing and service delivery.Demonstrated knowledge and/or experience providing information about healthcare services and resources available in the community.Demonstrated ability to establish effective collaborative relationships within and outside the organization.
