About Us: Reconnect Community Health Services is a not-for-profit health service organization in the west end of Toronto. Our services include supports for seniors, caregivers, and people living with mental health and addictions concerns. We pride ourselves on being at the forefront of client-centred care in our sector. Provides a full range of collaborative, team-based and client-driven services to clients of the COPA Program. The Case Manager's primary function is the provision of quality health care and support services through the effective and efficient use of resources. What You'll Do: You will be entrusted with a wide range of responsibilities, including: Initiates services to clients at the point of intake to ensure that proper assessment and initial service response occurs; Working with Older Adults and understanding aging specific complexities that include, but are not limited to, Physical Health, Cognitive impairment and Mental Health. Providing best practice-informed addictions counseling Develops clinically sound case treatment plans, using appropriate assessment tools, that are specific to the needs of clients and the objectives of the program in question; Ensures implementation and evaluation of the care plans; Ensures that all administrative functions related to her/his client list are completed; Establishes case management links with other resources (within both Reconnect and the community); Pursues case management activities independently and in concert with other professionals, agencies and groups; Maintains accurate and complete clinical records; Provides leadership in ensuring case conferences occur as required; Facilitates therapeutic support group sessions for clients; Provides support and education to family/friends/caregivers of clients or prospective clients; Collaborates with other appropriate service systems, organizations and individuals to ensure that services are well co-ordinated and meet a high standard of quality care and/or supervision; Identifies problems and possible solutions for resource use, and collaborates for service improvement; Assesses functional and resource needs in relation to the needs and goals of clients; Delivers all services in a professional manner and according to established guidelines and associated benchmarks; Follows all health and safety guidelines established by Reconnects Joint Health & Safety Committee; Other responsibilities may be assigned by the Program Manager. Technical and Behavioral Competencies Required: Completion of related Bachelors Degree or equivalent, Appropriate experience in a related field, Solid understanding of issues related to older adults with addictions/concurrent disorder and/or problem gambling issues, Demonstrated in-depth understanding of marginalization and vulnerability, specifically as related to older people living with a mental illness, addictions and chronic health issues. Demonstrated commitment to self-reflective and anti-oppression principles including bio psychosocial model, strengths model, harm reduction, Motivational interviewing and Stages of Change Excellent verbal and written communications skills, Proven history of working in multidisciplinary teams within the health and social services sectors Strong computer skills and able to learn new software applications, Experience in individual counselling and group work, Good problem-solving skills, Able to work independently and as part of a team, Able to work collaboratively with community resources, Excellent interpersonal skills, Good assessment skills, Familiar with relevant legislation (e.g., Mental Health Act, Personal Health Information Protection Act), Knowledge of and sensitivity to gender, race, sexual orientation, age and multicultural issues Special Workplace Requirements: Generally employees work a 37.5 hour workweek. Work hours may vary depending on the needs of the client. Must be willing and able to work flexible hours. Attendance at special events from time to time will be required.
