The Transition Care Program enables older people to have more time in a non-hospital environment to complete their restorative process and finalise access to their long-term care arrangements.The Home-based Allied Health team is a Commonwealth Home Support Program (CHSP) which focuses on restoring, or maintaining peoples independence, improving functioning and wellbeing to support them to remain living at home.
