Its specialities include cardiology, obstetrics and gynaecology, cancer, respiratory medicine, neurology, liver and kidney transplants.The Community Mental Health/Core Team model of care states:The Community Mental Health Team (Core Team) is the mainstay of the community mental health system. Core Teams provide a range of clinical services including intake, assessment, consultation, treatment, and care coordination. Care coordination is the main component of this model of care and includes extended care coordination, targeted packages of care, and short term care coordination.Consumers under the care of Core Teams will either have:Short term interventions and may be discharged or referred back to their GPs or other treatment providers after a period of weeks or months.A period of treatment as recommended by evidence-based practices.Ongoing care and monitoring for prolonged periods.The Mental Health Nurse will practice in keeping with the standardised Sydney LHD position description for the profession in addition to this position description.
