Want to empower seniors to live independently in their own homes and make a real difference to their lives?We have an exciting opportunity for an experienced Care Manager who is caring and compassionate, self-motivated, energetic and is looking for a position that has variety and offers an opportunity for ongoing professional development.About this role:Building relationships based on trust with clients and their families.Support a case load of clients with varying needs and levels of independence by managing and updating their Home Care Package (HCP) funding as required and ensuring that their changing needs are being met.Conduct regular quality assurance visits with clients in person and/or over the phone to ensure the highest level of care is being attained while also determining any changing needs or requirements for additional services.Ensure that clients changing needs and preferences continue to be supported by working together with the clients, client families, representatives and their caregivers.Be knowledgeable about a wide range of resources and local services so that you can inform clients and/or a clients family or representative about a range of tailored resources or services to meet any specific needs.Support, manage and lead the Client Experience Team by allocating new clients, monitoring care loads, and responding to queries.Monitor caregiver reports on a daily basis to identify any changes or concerns with the clients and pass onto any relevant parties.Ideally, we would like you to:Preferably have qualifications and experience in care planning, identifying needs, goal setting and managing care services within the aged, community health or social services.Be an effective communicator with excellent customer service skills and demonstrated problem-solving skills.Have a strong understanding and knowledge about the Australian Aged Care Standards, community home care and/or Home Care Packages.Be highly efficient and organised with proven time management skills.Have previous experience and strong skills in administration with accurate data entry and experience in the use of MS Office.Have a valid drivers license.What we offer:A rewarding career where you can make a positive difference in the lives of seniors and their families.Work laptop and mobile phone allocated to you.A friendly, supportive, and welcoming team that recognises and appreciates the work that you do.Comprehensive orientation, ongoing training and professional development program.Birthday leave - take a day off to celebrate your special day!Opportunity to work for a company that is locally owned and operated but also part of a national and global network.How do your skills match this job?Your application will include the following questions:Which of the following statements best describes your right to work in Australia?How many years'' experience do you have as a care manager?Do you have experience in administration?Do you have customer service experience?Do you have data entry experience?Do you have a current Police Check (National Police Certificate) for employment?Do you have a current Australian driver''s licence? #J-18808-Ljbffr
