We are committed to our vision of ''better health services for the people of western Sydney and beyond'' and to the delivery of health care or related services in line with our core values of collaboration, openness, respect, and empowerment.What you''ll be doing: The Hospital in the Home (HITH) Occupational Therapist (OT) is responsible for the provision of high quality, patient centred, evidence informed occupational therapy services that promote identified patient health outcomes. The OT will assess, plan, implement and evaluate the occupational therapy care plan in collaboration with individuals and the multidisciplinary HITH healthcare Service.The OT will provide clinical services to HITH patients, and work in collaboration with the HITH Senior OT in the provision of care for those with complex care needs requiring advanced practice skills and the application of professional knowledge and judgement.The OT will exercise professional judgement, and consult with the HITH Senior OT when required, in resolving problems and managing cases where principles, procedures, techniques and methods require expansion, adaptation and/or modification.The OT contributes to the planning, implementation, evaluation and reporting of HITH healthcare services to meet required Accreditation Standards and WSLHD Executive Strategic Plan.This position is expected to work from different locations across WSLHD and rotate across a 7-day, morning and afternoon service, as reasonably required.
