Location primarily remote and phone-based workCasual ideally 3 days minimum availability between Monday and Friday (flexible with days and hours)Pay rate $54.47 per hour + casual loading (25%) + superannuationPay less tax with Not For Profit Salary PackagingTravel will be reimbursed at 0.88 cents per km in line with our travel policy (if required for face to face assessments)Fitness Passport discounted gym membershipsOur Aged Care Assessment Services (ACAS) team is expanding, creating exciting opportunities! We are looking for compassionate and detail-oriented Clinical Aged Care Assessors with current AHPRA registration and a Statement of Attainment 3 in Advanced Assessment Practice.What will your role look like?In this role, you will be responsible for comprehensively assessing the health, functioning, and support needs of our elderly clients using the IAT assessment tool. Youll work closely with clients, their families, and service providers to develop individual support plans, with recommendations for appropriate Home Care Package levels, Residential Respite, and Permanent Residential Placement.Day to day this includes:Collaborating with clients, families, and multidisciplinary teams as requiredWorking collaboratively with colleagues to promote a supportive and positive team cultureConducting accurate aged care assessments using the Integrated Assessment Tool (IAT) to determine client needs and eligibility for home support servicesReviewing support plans to meet clients changing support needsWorking effectively both autonomously and as a member of a broader team in delivering to key performance indicators within identified timeframesMaintaining currency of knowledge relating to aged care services both funded and non-fundedAssisting with continuous improvement frameworks to regularly evaluate and enhance assessment processes and outcomesTo be successful in this role youll need:Tertiary Qualification in a health-related discipline directly related to Health, Aged Care, or related specialist area (e.g. Registered Nurse, Medical Officer, Occupational Therapist, Physiotherapist, or Social Worker)Current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA)Statement of Attainment 3, Advanced Assessment Practice (essential)An Australian Drivers license and access to a registered, insured vehicle (preferred but not essential)Willingness to apply for a Police CheckWhat we can offer you:Join a highly engaged dynamic team where youll be supported to grow and potentially gain ongoing permanent employmentMake a real difference in the lives of Older Australians play a critical role in assessing their needs and ensuring they get the right careBuild valuable skills and sharpen your expertise to help you grow a career in aged care servicesFlexibility and fulfillment where you can prioritize work/life balance, build strong relationships with your colleagues, and make an impact on clients and their familiesAged Care Assessment Services (ACAS) operate independently of Catholic Healthcare and play a pivotal role in ensuring that Older Australians are assessed for the care and services they truly need.If you''re passionate about making a difference and have the experience or skills for this role, we want to hear from you!Aged Care Assessment Services acknowledges the Traditional Owners and Custodians of the lands on which we live, work, and dream.
