About the role As the Care Manager at Kolan Gardens, Aged Care Service, you will be in charge of leading and mentoring a team of clinicians and care staff. You will work alongside the Service Manager in building and maintaining a culture of success. You will be required to complete 4 paper days per fortnight and 6 RN shifts on the floor as part of the Kolan clinical team. In this role you will: Coordinate and consult with residents around care planning and assessments in line with the positive wellbeing model of care; Review all clinical documentation and meet clinical KPI requirements on a monthly basis; Ensure associated legislative and external reporting requirements are met, including SIRS reporting as required; Complete performance discussions on an annual basis with clinical & care staff and assist with interviewing and onboarding of new team members; Attend regional clinical meetings either in person or online as required; Complete RN shifts on the floor as rostered. About you Reporting to the Service Manager, our Care Manager is responsible for the delivery of exceptional care to our residents. You will have a passion to build strong, meaningful connections with our residents and their families while modelling our "Positive Wellbeing Model of Care". This role would be ideal for an experienced Level 3 Registered Nurse with aged care experience looking for a new challenge. As the Care Manager, ideally, your experience and qualifications include: Registration with AHPRA as a RN with a current Practicing Certificate; Experience in managing and leading clinical care in the aged care sector; Strong knowledge of the aged care accreditation system and compliance; Demonstrated knowledge of AN-ACC; Hold Infection Prevention and Control Lead certificate or willingness to acquire qualification; Experience in educating, mentoring and motivating staff; Superior communication skills both written and verbal; Possess or, have the willingness to obtain, a National Police Check, National Insolvency Check, a current (CPR) and NDIS Screening Certificate.
