Job Category: Community Services & Development - Aged & Disability Support, OperationsOur purpose is to enrich the lives of people living independently in their own homes through connection to services and community.About ECHOECHO Community Services is a not-for-profit organisation providing care to the elderly to help them live in their own home with dignity while still maintaining their independence.We provide in-home services 7 days per week for Home Care Package clients and 5 days per week for CHSP clients.Our goal is to provide services at the same time, on the same day and with the same team of Support Workers which means that you know the people who are coming and the time they will arrive. Having the same support workers providing services, in particular, medication prompts and personal care, maintains the persons dignity and well-being. You know and can trust the people attending. There is continuity of care for people who receive our services.Job DescriptionRole:As an Enrolled Nurse at ECHO Community Services, you will play a key role in providing direct patient care by closely working with the clinical team. Your duties will include but not be limited to:Ensuring full compliance with and maintenance of aged care standards.Delivering clinical and other care needs in a safe and respectful manner that promotes clients'' personal values.Adhering to relevant legislation governing quality standards, professional clinical registration, and practice standards, including mandatory reporting responsibilities.Reporting and documenting any concerns affecting clients'' well-being.Assisting clients to meet their clinical and other care needs in a manner that promotes independence and choice in accordance with their individualised Care Plan.Accepting responsibility for specialist clinical portfolios including, but not limited to: continence, nutrition/weight management, palliative care, wound care, falls management, behaviours of concern, dementia, medication management, pain management, infection control, catheter and stoma care.Undertaking data analysis in respect to client care.Application of clinical care indicators to client wellbeing.Hours: Monday to Friday, 8:00 am to 4:00 pm, however, you must be willing to participate in a flexible roster involving extended hours.Desired Skills and ExperienceDesired Skills and ExperienceExperience in residential aged care preferred - new graduates with prior experience as a support worker are also welcome to apply.Registration with AHPRA as an enrolled nurse.Full vaccination against COVID-19 and Flu vaccination.A valid First Aid Certificate (Ongoing Renewal required).A satisfactory National Police Clearance Certificate obtained within the last 6 months.Have a reliable car with the willingness and confidence to drive in the metro area.Demonstrated proficiency in Microsoft Office Suite applications.Competency:Strong person-centred communication skills and behaviours, particularly with clients with dementia and/or high clinical needs.Ability to assess, plan, implement, and evaluate client care in consultation with a multidisciplinary team.Strong analytical and problem-solving skills, with the ability to prioritize tasks, work under pressure, meet deadlines, and use resources efficiently.Knowledge and understanding of continuous quality improvement, quality standards, and accreditation processes.Excellent interpersonal skills and the ability to communicate with diverse backgrounds and experiences.Qualification in Dementia care and behaviour support or significant experience.Qualifications in Palliative Care.Make a difference with your clinical skills - apply now and join our caring team! #J-18808-Ljbffr
