Our focus is on collaboration, empowerment, and person-centred support, ensuring each participant receives the care and opportunities they deserve.About the Role:As an NDIS Team Leader, youll work closely with the NDIS Service Delivery Manager to oversee service delivery, ensure smooth transitions for participants, and support the professional development of our team. You will be the key point of contact between participants, families, support workers, and external agenciesensuring seamless communication and quality service provision.This role also involves on-call responsibilities, administrative tasks, compliance monitoring, and rostering. If youre passionate about making a real impact in peoples lives, wed love to hear from you!Main Responsibilities:Develop, implement, and monitor individualised care plans for CA/SIL/SDA participants.Act as the primary contact for participants, families, and key stakeholders.Oversee participant onboarding, ensuring documentation and service agreements are completed.Coordinate community access activities and participant transitions into CA/SIL/SDA services.Provide guidance and support to support workers, assisting with complex cases.Assist in staff training and development to enhance service quality.Conduct weekly house visits and inspections to maintain high care and safety standards.Fill shifts as needed, including overnight sleepover shifts, ensuring continuity of support.On-Call & Rostering:Participate in the on-call roster, responding to urgent staffing and operational needs.Adjust and reassign shifts, ensuring compliance with employment conditions.Approve leave requests and maintain adequate shift coverage.Provide after-hours support and scheduling assistance for workers.Compliance & Reporting:Maintain Medication Health Passports and ensure medication records are accurate.Conduct and update risk assessments every six months or post-incident.Oversee incident reporting and ensure compliance with NDIS and WHS regulations.Prepare monthly reports on participant progress, incidents, and program evaluations.Ensure all care plans and compliance documentation remain up to date.Skills & Experience:Strong understanding of NDIS principles, disability rights, and support services.Leadership experience in NDIS, healthcare, or disability support.Person-centred approach with experience in individualised support planning.Proficiency in Microsoft Office and relevant software platforms.Exceptional organisational and problem-solving skills, with the ability to prioritise tasks effectively.Excellent communication skills, with the ability to engage with diverse stakeholders.Ability to work both independently and collaboratively, demonstrating initiative and professional judgment.Alignment with our core valuesintegrity, accountability, and collaboration.Mandatory Requirements:Proof of citizenship (passport, birth certificate or citizenship certificate) or VISA if applicable.Must hold a Working with Children Blue Card.Hold a current First Aid (HLTAID011) and CPR (HLTAID009) Certificate.A clean National Police check less than 6 months old.Certificate III in Individual Support or equivalent.Willingness to undergo training in specific courses to support the participants individual needs.Ability to communicate effectively with clients, their families, community agencies, and other professionals.Ability to work in a busy environment, both independently and as part of a team.Comply with & understand all privacy and confidentiality policies.Cultural awareness of all backgrounds.Why Us?Essence of Care:Immerse yourself in the heart of care at Enabling Pathways, where compassion extends beyond the workplace to directly impact the lives of our clients.
