If you have just the right amount of crazy, lace up your boots and join our revolutionary new approach to healthy communities.What we offer:Ongoing training and development, including a comprehensive onboarding programREFOCUS uniform, so deadly, you''ll want to wear it on your days offSalary sacrificing, up to $15,900 saving each year tax free, and moreUse of REFOCUS fleet vehicle to transport participantsThe opportunity to work for an organisation that is daringly disrupting the system and searching for like-minded renegades to join our ranksTransforming and empowering communities to enjoy longer, healthier lives is our missionPrimary Role: NDIS Support CoordinatorWork Location: Nambour, Sunshine CoastEmployment Type: Part time, 4 day work weekSalary Range: $35 - $43 p/h - subject to award rateQualification & Experience: Relevant Diploma or Degree AND/OR Minimum 2 year experience in the disability support sectorDuties:Develop trusted, positive and professional relationships with participants and their support workers.Support individuals and their nominated stakeholders to understand the supports in their plan and navigate the service system, identifying and accessing mainstream, community, informal and funded support.Negotiate services and rates, in accordance with NDIS guidelines and an individual''s budget.Develop service agreements and create electronic service bookings.Strengthen and enhance participants'' capacity to coordinate supports, self-direct and manage supports, and participate in the community.Be the key contact for service issues, complaints, major changes, and plan reviews.Case note activity, monitor individual outcomes and satisfaction with service provision, and review expenditure.Attend immediately to any complaints submitted by participants and work collaboratively with teammates to resolve any concerns and/or conflicts.Please provide details of your knowledge and experience in:Supporting NDIS participants to understand their plans and help them connect with services to improve their quality of life.Client-centred practice.Knowledge of service providers, support options, and community networks available for people with disabilities.Applying a sense of cultural competence being aware of cultural sensitivities and respecting cultural differences.COME ON YOU MOB HAVE A GO!Only successful applicants will be contacted. #J-18808-Ljbffr
