An integral part of our Disability Employment Services team, the Job Coach is responsible for supporting the achievement of high quality employment outcomes for young people with a disability through delivery of UnitingCares Project SEARCH Transition-to-Work Program.The Project SEARCH program is a unique, business-led, one-year employment preparation program that takes place entirely at a Host Workplace. Total workplace immersion facilitates a seamless combination of classroom instruction, career exploration, and hands-on training through worksite rotations and provides young people with an intellectual disability successful and supported entry into the workforce.The purpose of this position is to provide culturally competent case management services to assist young people with a disability to build their work capacity and attain the skills required to achieve their employment related goals. This will include ensuring that service provision aligns with practice frameworks and legislative, regulatory and licensing requirements, developing a range of meaningful internship opportunities at the host site, undertaking Intern recruitment and on-boarding activities, delivering pre-employment and job skills training, facilitating goal progression sourcing suitable post program employment opportunities, developing and maintaining effective stakeholder relationships and contributing to the development of quality practice to ensure the effective co-ordination of service delivery to vulnerable individuals and families.This position may be required to work across a number of program sites within a designated geographical location.Your part in our UnitingCare Community team:Deliver services in accordance with the Project SEARCH Model Fidelity, practice frameworks and legislative / regulatory / licensing requirements ensuring that key deliverables, goals and objectives are achieved within prescribed time frames.Develop and maintain effective networks, relationships and partnerships both internally and externally with all Project SEARCH stakeholders including, Host Site department managers / mentors, co-workers, family members, government bodies, employers, community and other organisations that will support the achievement of quality client outcomes.In partnership with the leadership team and host sites, develop a range of internships designed to teach Interns competitive and marketable skills that are transferable to jobs within the community.Work with the leadership team to identify, assess and select eligible young people with a disability to participate in the Project SEARCH Transition to Work program.Complete a range of assessments to determine Intern capacity, support requirements and areas for skill development in order to facilitate the achievement of each Interns employment related goals.Provide employment preparation training and a range of vocational and non-vocational interventions to assist Interns to manage employment related barriers and develop the skills, confidence and attributes necessary to successfully complete each internship rotation and transition into open employment upon program completion.Provide individualised on-the-job support to each Intern during the internship rotation to facilitate the development of competitive and marketable skills.Achieve Job Placement Key Performance Indicators by undertaking employer engagement activities to secure post program employment opportunities for interns.Build and maintain relationships with follow on services such as Disability Employment Service providers to assist each intern transition in to open employment and receive ongoing employment support upon program completion.Maintain data collection processes and information systems to ensure quality and responsiveness of service delivery including timely completion of a range of administrative and reporting functions relevant to the role with a high level of accuracy and attention to detail.Work within relevant quality and risk frameworks and guidelines in accordance with NDIS Practice Standards to ensure delivery of high-quality services, that are respectful of clients rights and confidentiality.Work together with the leadership team to support innovative and responsive service delivery for young people with a disability and contribute to the development of the programs plans, practices, policies and procedures.Contribute to the achievement of UCQ strategic directions within Program.Undertake other duties as and when directed within the scope of the role and the capabilities of the incumbent.What makes a difference for us?Certificate IV in Employment Services, Disability Services, Community Services or other relevant qualification.Lesser formal qualifications with acquisition of considerable skills and extensive and diverse relevant experience to an equivalent standard.Current Drivers Licence.Positive BlueCard.Positive NDIS Worker Screening Check.Senior First Aid Certificate.Demonstrated experience in successful, culturally sensitive case management and planning practices related to people with a disability, with a focus on individualised assessment, employability skills development and successful transition into the open labour market.High level written and verbal communication skills demonstrated by the ability to relate well to all people, work as part of team and maintain accurate records.Demonstrated ability to plan, organise and manage workloads effectively ensuring key deliverables are achieved within set time frames.Demonstrated ability to use initiative, make well informed decisions and problem solve.Ability to work effectively and constructively across a range of stakeholders, including the business sector, community members and organisations.Existing, robust connections within the sector, capable of opening doors and securing post program employment opportunities for Interns.Demonstrated understanding of the NDIS environment, funding and reporting models, service delivery models, and quality service frameworks.Demonstrated understanding of key legislation and standards that exist at the Federal and State levels that impact on service delivery for Disability and Employment Services.What makes a
