Melbourne City Missions service profile includes Youth, Adult and Family; Disability; Employment and Education; Homelessness; and Palliative Care.The Community Integration and Accommodation Options - CIAO program serves young people under the age of 18 and aims to assist them in developing the skills necessary for healthy and sustainable, independent living within their community of choice. CIAO facilitates a beneficial and meaningful transition from Out-of-Home Care (OOHC) toward self-sufficiency. Using a holistic approach, the CIAO program targets interventions to the many facets of adolescent development and wellbeing. The CIAO Lead Tenant program comprises four properties in Melbournes Northwest, with each property housing 2 young people and 2 live-in volunteer mentors.The Case Worker will provide support and services to young people in the Lead Tenant program who are transitioning from the OOHC system into independent adulthood.About YouWe are looking for someone who can:Provide outreach support to young people in the Out-of-Home Care system who have experienced trauma and exhibit a range of complex and diverse behaviours.Provide culturally safe responses and personalised service to young people based on their unique circumstances and needs.Collaborate with external and internal stakeholders, within a multi-disciplinary framework, to enhance service delivery to young people.Champion trauma-informed, anti-oppressive, and inclusive approaches to working within the team, co-managing clients and supporting other colleagues in providing quality supports to highly complex young people.Ensure service delivery is consistent with current legislation and best practice interventions.Benefits of Working with MCM Include:Inclusive Employer by the Diversity Council of AustraliaAttractive Salary Packaging benefitsSupportive learning and development environmentCulture of continuous improvementMinimum Requirements:Relevant tertiary qualification in Community, Social Work, Youth Work or related fieldDemonstrated knowledge of adolescent development and Trauma Informed CareExperience and understanding of service systems, relevant legislation and practice including Child Protection, disability, drug and alcohol, physical well-being, housing, and mental health servicesAbility to establish and maintain cohesive working relationships with a broad range of networksHigh level of communication skills, organization and time management skills, including technological competencyDemonstrated ability to work autonomously and as part of a teamExpertise in case management, including assessment and planning processes; ability to actively and assertively engage young people with complex needsScreening Criteria:National Police checkWorking with Children checkValid Victorian Drivers licenceRight to Work in AustraliaApplications for this role will close on 24th February 2025 (or applications may close before the deadline, so please apply early).
