You will provide clinical pharmacy services to inpatients and assist in our dispensary ensuring that all pharmacy activities are completed to a high standard while supporting the quality use of medicines.This is an exciting opportunity to further enhance your career in an ever-evolving multidisciplinary team environment. This role would suit either someone with previous hospital experience wanting to use their existing clinical skills or someone looking to break into hospital pharmacy.About the RoleIn this role you will rotate through wards and dispensary. Your responsibilities will include:Efficiently and effectively dispensing and distributing medication.Conducting medication history interviews, performing medication reconciliations (Medication Management Plans), and preparing discharge summaries during transitions of care.Offering medication counselling and education to patients.Collaborating with multidisciplinary teams to provide advice on patient care.Contributing to training and educational sessions for pharmacy staff, nursing staff, medical personnel, and allied health professionals.Assisting in overseeing imprest services and ensuring the continuity of medication supply.Considering the financial implications of medication use both within the hospital and for patients obtaining medicines in the community.Rostered hours: Tuesday to Saturday, earliest start is 7.30am, and latest finish is 7.00pm.About YouWe welcome all Pharmacists to apply, whether youre an experienced Hospital Pharmacist or an aspiring Community Pharmacist looking for your first Hospital Pharmacy experience.
