Improving the connection, pathways and collaboration between Mental Health Services and Homes NSW to achieve better outcomes for consumers in accessing and/or sustaining tenancies.Utilising their expertise in mental health and housing models and pathways to support complex care coordination to ensure community tenure and:access public and social housingmaintain tenancies through support packages such as the Housing and Accommodation Support Initiative (HASI) and Community Living Supports (CLS) program.Liaising with key Mental Health teams, Housing teams, and community managed organisations (CMOs), project management, and consultancy.
