HammondCare provides care primarily through hospitals, residential care homes and home-based care as well as services for dementia support, palliative care, rehabilitation, positive aging and older persons mental health.About the roleReporting to the Regional Learning Team Leader, you will work within the Mission in Action of HammondCare and be part of the People and Learning Team who provide a high performing and valued People and Learning function.The Hammond Care at Home (HCAH) Workplace Coach reports to a Regional Team Leader Workplace Coaches and their focus is to work in consultation with the Care Manager and Learning Team. They focus on improving the lives of people receiving HammondCare services through delivering consistent, high-quality training and coaching to HCAH Care Workers. Their key objective is to empower Care Staff to operationalise the HammondCare Model of Care.This is a permanent part-time role working either 3 or 4 days per week.
