Pathways Home Health & Hospice may ultimately pay more or less than the posted range as permitted by law.POSITION SUMMARY:The Hospice Resource Registered Nurse is responsible for the coordination of service delivery during regular business hours to Hospice patients, families, and facility staff. Provides clinical and administrative coordination for clinical and office staff. Works from the office.AREAS OF RESPONSIBILITY:Triages incoming calls from patients andor their family members. Assesses information provided and recommends appropriate nursing interventions. Consults with Interdisciplinary Group (IDG) members as indicated to determine appropriate interventions to be taken and makes recommendations. Accepts incoming phone calls from members of the community inquiring about hospice services. Provides appropriate information. Accepts and documents physician verbal and written orders and obtains other patient care information from physicianpatientfamily. Communicates new ordersinformation to the case manager or appropriate IDG member, including pharmacy providers. Provides clinical consultation as needed to clinical providers and office staff. Maintains current knowledge of regulations pertinent to the delivery of Hospice and Home Health care services and demonstrates compliance with such regulations in practice. Knowledgably discusses overall history, philosophy and services of Pathways Home Health and HospiceAccesses electronic record during patient communicationinteraction and documents during or immediately after the communicationinteraction per agency policy. Maintains the dignity of the dying patient and supports the patient's and family's unique spiritual and cultural beliefs. Participates in the hospice program's quality assessment performance improvement program and in-service education related to areas of practice as appropriateMeets productivity standards as established by the manager. Maintains timely communication with patient, family, and facility staff regarding all aspects of patient careMaintains and updates level of knowledge and skills required to provide quality hospice nursing careProvides assistance with ordering medications and communication with pharmacies. Effectively communicates with and uses Delta and the formularypreferred drug list, and general hospice medication coverage philosophy. Adheres to all agency policies and procedures, including updates, as communicated in written, spoken, and electronic formsPromotes a culture of service when representing Pathways by at all times treating internal and external contacts with respect, remaining professional, communicating clearly, preventing conflict, following through on commitments, and actively listening. Positively promotes Pathways by offering assistance and informationAssist in auditing Quality Reports as requestedCovers for the Manager of Clinical Services (MCS) in their absenceAssists in development of hospice protocols, procedures and policies as requestedDemonstrates all skills of a Registered Nurse, including those skills required by the agency through competency testing. Performs other duties as assigned consistent with skills and training and the mission and goals of PathwaysQUALIFICATIONS:California RN license requiredRecent experience in hospice requiredMinimum of two years of nursing experienceMust be highly motivated, self-directed, flexible, and able to work well and participate in a multidisciplinary team setting.
